About Aged Care GP

Aged Care GP is a national aged care medical service that supports General Practitioners caring for people living in residential aged care facilities. Working across South Australia, Victoria and Queensland, Aged Care GP partners with more than 100 facilities to deliver consistent, proactive medical care to residents who are unable to attend a traditional general practice.

With over 15 years of experience in the aged care sector, Aged Care GP has grown into one of the largest providers of this specialised model of care. Their approach is focused on supporting doctors to deliver high-quality care, while the business manages the operational and administrative complexity that comes with delivering care across multiple facilities.

Delivering medical care in aged care is very different to running a traditional general practice. Doctors work across multiple facilities, residents move between services and, with the introduction of the General Practice Aged Care Incentive (GPACI), eligibility now needs to be tracked across different patients and quarters.

Before Cubiko, much of this relied on manual reporting and spreadsheets. Creating accurate, doctor-specific insights was time-consuming and still left room for missed opportunities. The team needed a clearer, more reliable way to see who needed to be seen, what actions were required and when, without adding to the administrative burden.

We had the pleasure of catching up with Kate Dalgleish, Operations Manager at Aged Care GP, to talk about how the team has used Cubiko to simplify complex aged care requirements, reduce manual workload and support more informed decision making across the business.

Turning complex requirements into clear, actionable insights

Cubiko has worked closely with Kate and Aged Care GP to help translate complex aged care and GPACI requirements into clear, practical insights that doctors can easily act on. Rather than relying on manual tracking across spreadsheets and reports, the team now has data that clearly shows who is eligible, what services have been delivered and how each resident is progressing through their current quarter and 12-month GPACI cycle.

For Kate, clarity was key to engaging doctors. As she explains, “What the doctors want is a clear list that shows which patients need to be seen, where they’re up to in their care cycle and what services they’re eligible for next.

By presenting this information in a simple, prioritised way, these insights help the Aged Care GP team support doctors to deliver care, without needing to manually interpret complex eligibility rules for each individual patient. As Kate puts it, “Cubiko takes complex requirements and turns them into a clear list for doctors to work from.

Saving time and reducing manual workload

Before Cubiko, producing accurate, doctor-specific insights was a highly manual process for the Aged Care GP team. Reports needed to be run across multiple systems, exported into spreadsheets and carefully checked to make sure nothing was overlooked.

The first time I did it, it took me three full days,” Kate says. “Even once I got faster, there was still a lot of hands-on effort to make sure everything was captured correctly.” Despite the time involved, the team still couldn’t be confident that nothing had been missed.

With Cubiko’s GPACI data, this process is now significantly faster and more reliable. What once took days can now be done in a matter of hours, giving the team confidence in the data and freeing up time to focus on supporting doctors and improving workflows.

Greater visibility to support planning and decision making

Cubiko has also given Aged Care GP greater visibility across key operational and financial metrics, allowing the leadership team to regularly review performance, monitor trends and identify potential issues early.

For Kate, this level of visibility supports more informed conversations and decision making. “What you want is visibility and transparency. You want to be able to see what’s happening at a glance,” she says.

By bringing key insights into one place, Cubiko helps the team stay aligned and proactive as the business continues to grow. “Being able to see that overview helps us pick things up early, before they turn into bigger problems,” Kate adds.

With Cubiko supporting the operational side of aged care delivery, Aged Care GP is better equipped to support doctors and ensure residents receive consistent, high-quality care. As the organisation continues to grow, having clear, reliable data in place helps the team stay focused on what matters most, supporting doctors and improving outcomes for residents.
