Join Elisha and Ros as they catch up and chat about how practices can use Cubiko’s Indigenous Health workflow to identify care opportunities, support proactive patient outreach and improve health outcomes for Aboriginal and Torres Strait Islander patients.
In this session you will:
- Identify Aboriginal and Torres Strait Islander patients who may be eligible for key services and initiatives, including Health Assessments, Chronic Condition Management, Mental Health services, vaccinations and support programs.
- Use Cubiko’s Indigenous Health workflow and metrics to create a proactive, team-based approach to patient identification, outreach, follow-up and ongoing care delivery.
- Monitor and improve Indigenous health outcomes using Cubiko’s metrics and dashboards to track ethnicity recording, Closing the Gap registrations and service delivery opportunities, supporting quality improvement activities and better patient outcomes.

Will I receive a Certificate of Attendance or an AAPM CPD certificate for attending this webinar?
Yes, all attendees will receive a Certificate of Attendance, which can be added to your practice’s training register for accreditation purposes.
However, unless the webinar description specifically states it is eligible for AAPM CPD points and the landing page displays the AAPM CPD Approved badge, this webinar does not qualify for AAPM CPD points.
