One Care Family Doctors (OCFD) is a well-established general practice that’s been serving the Upper Mount Gravatt area since 1977. With a comprehensive range of services including preventative care and chronic disease management, OCFD is committed to providing healthcare to all patients within their community.

We sat down with Liz and Amber Meaclem, the practice manager and the 2IC of OCFD, to learn more about what life was like before Cubiko and the impact Cubiko has had on their practice.
Being a well-established practice, OCFD has seen the evolution of data collection in General Practice. From manually going through paper patient and doctor records to extracting data from their PMS, OCFD has seen it all. OCFD needed an automated system that would help them:
OCFD’s 2IC Amber Meaclem has always been interested in the data found within Cubiko. Coming out of the other end of COVID the team at OCFD made the decision to form a committee that focuses on using the data within Cubiko to identify any blind spots within the practice, as well as, opportunities for improvement.
Coming out the other end of the COVID-19 Pandemic the team at OCFD wanted to drive up their preventative health initiatives by getting patients back in for their Chronic Disease Management (CDM) and Health Assessment consults. “We used Cubiko to download lists of patients that may be due or eligible for care plans and started booking them in for appointments” says Amber.
The use of Cubiko has had a remarkable impact on helping OCDF get back on track with their preventative health initiatives. “Over the last three months we’ve seen a significant increase in the number of care plans performed… just looking at the numbers we’ve completed four times as many care plans than we did this time last year”.
Not only has Cubiko helped them get back on track with their health initiatives, but it’s also had a significant impact on their patients. “With the help of Cubiko we were able to identify a patient who was due for a health assessment. “Life before Cubiko was very manual, a lot of the statistics we were collecting had to be done manually which was very time consuming” says Liz Meaclem. Since implementing Cubiko in their practice the time spent on reporting has been significantly reduced. Giving both Liz and Amber more time in their day to focus on the areas of the practice that are most important to them and the practice.
“I’ve always been interested in the data found within Cubiko. I would often relay my findings to management who became very interested in the insights I was finding” says Amber. “I was finding blind spots and areas within the practice that could be improved upon, and soon we formed a committee to focus on Cubiko and the benefits that it can have on the practice”.
Cubiko helped One Care Family Doctors get back on track with their preventative health campaigns. Helping them to complete 4 times as many care plans than this time last year.
Using Cubiko’s Item Eligibility reports One Care Family Doctors is able to identify opportunities to deliver care and achieve greater health outcomes for their patients.
